Our Consultants and Staff "get" the small details that make a difference whether that be for short-term home help or from 1 hour to 24/7 personal support.
Our key focus is to support independence and choice for clients through a person- centred strengths-based approach. In developing care packages, we explore and partner with the client, their family/whanau and trusted network to co-author what their care plan/pathway will look like.
By combining this information with a full clinical assessment of client needs and risk assessments, and considering any cultural needs/wishes we are able to create a complete picture of the client that enables us to select appropriate trained, qualified and experienced staff to deliver a holistic care package designed to respect client choice, support maximum independence and quality of life.
Life Plus is a direct contractor to ACC for Living My Life Disability Services and a contractor to Solora for ACC Home and Community Support Services.
